If you are a JLA member but are not able to attend the special meeting of members on October 5th at 7:30pm, please register your vote using this (PDF) fillable proxy form.
  • Type your full name at the top
  • Choose your voting options
  • Type your full name on the Signature line
Instructions on where to send your completed proxy are included on the form.

Note:  Deadline to submit a proxy form is 12pm (noon) on Tuesday October 5th.

If you are joining the meeting by phone, please complete a proxy form in advance as you will not be able to participate in the live vote on an audio-only connection.
